Care Plans and Health Office Forms
If your child is diagnosed with a medical condition, please download, print and fill out the appropriate Health Care Plan (dated after June 30th of that school year). Bring this to the nurse with a picture of your child attached. Copies of this care plan will be given to the principal, classroom teacher, specialists and the cafeteria if needed.

Any child that will need medication at school will need to have a doctor's order (dated after June 30th of that school year) and the parent/guardian must fill out the form attached to give the nurse authorization to give your child medication. Also dated after June 30th of that school year.
Reminder: Any medication, prescription or over the counter, can NOT be sent in with your child! It MUST be brought in by an adult!
